Applying to CSOD is easy. Our elementary and secondary school applications are available for download and can be printed from home. Please do not hesitate to contact us with any questions.

 

Preschool

Elementary School

Elementary Application

Elementary applications require one academic reference to be filled out by an English teacher and one character reference.

 

Secondary Application

Secondary applications require two academic references and one character reference. One academic reference must be filled out by an English teacher.
